Project Details
The Utah CARE (UCARE) project, funded by the Utah State Department of Health and Human Services (DHHS) and the Centers for Medicare and Medicaid Services (CMS), researched and developed resources and services to support caregivers who take care of individuals with special health needs. UCARE implemented the Train the Trainer program for the Real Choice Program and developed a website that offers information, resources and training information for caregivers.
The UCARE Caregiver's Curriculum is available through trained trainers to teach the UCARE curriculum as a resource and support for informal caregivers of the aged, chronically ill, and persons with disabilities or mental illness of all ages in multiple locations across the state of Utah.
