CFS Helps Children with Disabilities Find Healthcare Services


It can be especially difficult to locate services for children with special healthcare needs for economically disadvantaged or minority families. The Coordinated Family Support-Medical Home project (CFS) is a partnership between the CPD and the IHC Budge Clinic Pediatric Unit that is designed to provide enhanced medical care to children with special healthcare needs in the Cache Valley and surrounding rural areas. The CFS-Medical Home team provides a variety of services to families throughout nine counties in Utah, Idaho, and Wyoming.

In order to better serve the needs of the Latino community and culturally diverse families, the project team recently added a Spanish-speaking care coordinator. CFS-Medical Home can refer families to additional resources in their community, provide translation services, and assist them in applying for a variety of programs.
